Possible fan problem

Is the fan suppose to run all the time or does it not start up until machine gets warm??

Depends on what fan your talking about.

Cpu all the time or cpu will burn out or shut down.
Video card same thing fan is needed to continuously cool the GPU.
Power supply all the time or it will overheat and fail.
Case or auxilliary fans usually all the time to keep an air flow within case to cool components.
CDrom fan newer ones have a cooling fan which kicks in when cd is being read and sound like similiar to an airplane getting ready to take off.
With faster (above 24X) don't leave a cdrom in the drive that way the fan won't start intermittently.

Some of these fans can change speed such as some cpu fans will change speed based on temperature readings.

Some versions of Windows check the CDrom intermittently and thus kick in the loud fan.
