This modem is made by GVC (SF-1156HV+/R19) or by KTX (56KVSE).
It is a 56k internal, ISA
What I need is the jumper settings or manual on this particular modem.

Went to the link suggested with no problem. Both the manual and driver was listed. You need to download the modem driver file and read the readme.1st file.

The manual has no jumper settings.
Will have to look else where.

Peter, I have the /R21 version of this modem and the jumper settings are included in the manual, your /R19 model should be fairly close as far as jumper settings are concerned as they both use the Rockwell:Conexant chipset. The main concerns with these modems are setting up ports while the existing comports are still enabled on the mainboard I.E. serial Port 1&2 in bios. If you need help in this area post back here and I can walk you through it.

Could you please send an email attachment with the jumper settings as I should be able to work them out.
Email address is supplied with this message.
